Hemorrhoids (affects 75% of people at some point). Anal fissure from straining or hard stool. Small polyp that's bleeding but not dangerous. Vigorous exercise.

Bright red blood on the toilet paper or in the bowl after a hard stool is almost always hemorrhoids or a fissure. Dark, tarry stool is different — that suggests bleeding higher up in the GI tract and is more significant. Context, color, and consistency all matter.

⚠️ When to actually call your doctor

These are real red flags. If any of these apply, don't wait.

  • Dark, tarry, or maroon-colored stool (not bright red)
  • Significant blood — more than just streaks
  • Accompanied by weight loss or change in bowel habits
  • Over 45 and not had a recent colonoscopy
  • Persists beyond a week or keeps recurring
  • Abdominal pain with the bleeding
